How to use the Care Passport
Fill in the Care Passport to be prepared in case of a medical emergency.
Handing this document to medical professionals or emergency room staff will save you valuable time.
For safety:
- use the Care Passport
- You can click on the button on this page and log in. You will be taken to your personal research page and the Care Passport will be ready for you to fill in.
You will be asked to fill in some questionnaires, indicate medication, allergies and/or treatments.
Print the Care Passport
- Keep a copy in your home; give a copy to your child's school or day care centre and keep a copy with your child (in a rucksack or carrier bag)
Update it regularly
Make sure to update any changes in medication/conditions/doctor information as needed.
