A macro to create the research Header

A Research header has two states:

• the user has not yet decided to participate (participate is null or participate is false)
• the user has decided to participate

Not participating

(the text appearing in the consent statement is in a translation page)
There should be a header like the image below:

Note: that the button pointing to the questionnaires does not appear
Note: that the hyperlinks on each questionnaire to enter the questionnaire is not created
Note: the macro will only generate the blue rectangle and the content inside
